Allied Pathways ("we", "us", "our") is committed to protecting your privacy and handling your personal information responsibly, in line with the Australian Privacy Principles (APPs) under the Privacy Act 1988 (Cth). As a health service provider, we take our privacy obligations seriously regardless of our size — health service providers are not covered by the small business exemption that applies to some other businesses.
This policy explains what personal information we collect, how we use and store it, who we may share it with, and how you can access, correct or raise concerns about your information. It applies to information collected through our website, our referral and intake process, and the allied health and nursing services we deliver.
What we collect
Depending on how you interact with us, we may collect:
- Contact details — name, phone number, email address, and postal or home address.
- NDIS and funding information — NDIS participant number, plan details, plan management type, Support at Home package details, and similar funding information relevant to your referral.
- Health information — clinical and health-related information relevant to the services we provide (for example, dietary needs, swallowing or communication needs, mobility, nursing care needs, and relevant medical history). This is "sensitive information" under the Privacy Act and we handle it with extra care, generally only with your consent or as necessary to safely deliver your care.
- Referral and communication details — information you, your family, or a referring professional (such as a Support Coordinator, GP, or Care Partner) provide when making a referral or enquiry.
- Website usage information — we use Google Analytics, which collects general technical information such as your approximate location, browser and device type, and pages viewed, to help us understand how the website is used.
How we collect it
Most personal information is collected directly from you — through our contact form, referral form, testimonial submission form, meal delivery audit enquiry form, by phone, email, or in person. Where you're referred by someone else (a family member, Support Coordinator, GP, or Care Partner), we may collect information from them on your behalf, with your consent where required. Some technical information is also collected automatically by Google Analytics as you browse.
Why we collect and use it
We collect and use personal information to:
- Assess and respond to referrals and enquiries;
- Provide allied health and nursing services, including clinical assessment, treatment planning, and progress reporting;
- Communicate with you, your family, and (with consent) other professionals involved in your care;
- Meet our funding, billing and regulatory obligations, including as a Registered NDIS Provider;
- Maintain accurate clinical and business records; and
- With your explicit consent, feature testimonials or case studies (never treatment outcomes, in line with restrictions on advertising regulated health services).
Who we may share it with
We don't sell your personal information. We may share it, only as needed, with:
- The NDIA, your Plan Manager, Support Coordinator, or aged care Care Partner, where relevant to funding or coordinating your care;
- Other treating professionals involved in your care, with your consent;
- Our practice management and referral intake platform (Splose), which processes referral form submissions on our behalf;
- Our website hosting provider, for the technical operation of this site;
- Google, as our website analytics provider (Google Analytics); and
- Regulators or authorities where required by law, such as the NDIS Quality and Safeguards Commission.
Some of these providers (for example, Google) may store or process information on servers located outside Australia, including in the United States.
How we store and protect your information
We take reasonable steps to protect personal and health information from misuse, loss, and unauthorised access, including restricting access to authorised staff and using secure systems for clinical and business records. No online transmission or electronic storage system can be guaranteed completely secure, but we work to minimise this risk.
How long we keep it
We retain personal and health information only for as long as necessary to provide our services and to meet our legal and regulatory record-keeping obligations as a health service provider and Registered NDIS Provider, after which it is securely deleted or de-identified.
Accessing or correcting your information
You can ask us for access to the personal information we hold about you, or ask us to correct it if it's inaccurate, out of date, or incomplete. Contact us using the details below — we'll respond within a reasonable timeframe and may need to verify your identity first.
Privacy complaints
If you have a concern about how we've handled your personal information, please contact us first using the details below — see our Feedback & Complaints page for how this is handled. If you're not satisfied with our response, you can also contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or on 1300 363 992. As a Victorian health service provider, you may also contact the Health Complaints Commissioner (Victoria) at hcc.vic.gov.au or 1300 582 113 regarding health-related privacy or record-keeping concerns under the Health Records Act 2001 (Vic).
Data breach notification
If a data breach occurs that is likely to result in serious harm, we will notify affected individuals and the OAIC in line with the Notifiable Data Breaches scheme under the Privacy Act.
Cookies and website analytics
This website uses Google Analytics, which sets cookies to help us understand how the site is used (for example, which pages are visited). It does not collect the contents of forms you fill in. You can control or disable cookies through your browser settings, or opt out of Google Analytics entirely using Google's opt-out browser add-on.
Changes to this policy
We may update this policy from time to time as our practices, technology, or legal obligations change. The current version will always be available on this page.
